Where can I Find Plan Benefit Information?
Details of your benefit information can be found in the Summary of Benefits and Evidence of Coverage documents for your plan. To access these documents visit our home page and choose your state and your county. Then visit our Explore Your Plan section and select the name of your plan in the left-hand column. There you will find links to view the complete Summary of Benefits, Evidence of Coverage, and other important plan documents.
